How to Submit a DMCA Takedown Notice
A valid DMCA request must include the following information:
- Your full name and contact information (including email and phone number)

- Identification of the material that is claimed to be infringing (including specific URLs or channel names if applicable)
- A statement that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law
- A statement that the information in the notice is accurate, and under penalty of perjury, that you are authorized to act on behalf of the copyright owner
- Your physical or electronic signature
