How to Submit a DMCA Takedown Notice

A valid DMCA request must include the following information:

  3. Identification of the material that is claimed to be infringing (including specific URLs or channel names if applicable)
  4. A statement that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law
  5. A statement that the information in the notice is accurate, and under penalty of perjury, that you are authorized to act on behalf of the copyright owner
  6. Your physical or electronic signature
